How to Send Online Forms

How to send general / consent forms to patients

If your patient needs to fill out some paperwork or be sent a consent form, you can send it to them through Practice by Numbers. The easiest ways to do so is either through the Communication Center or the Patient Information Window.

If you are looking to automate the messages, you can do so with Patient forms, but Consents have to be sent manually as in most states the consent needs to be completed in the presence of the doctor.

You can find the Communication Center at the bottom left of every page in Practice by Numbers. Click on the option to open it up, and it will pull up on the left.

With the Comm Center open, next is to select the patient who you need to send the information to. You will see all the recent open conversations on the left, but to start a new thread you can search for them using the Compose option at the top right of the panel. This will open a search bar for you to input their name.

Once you select the patients name, it will open up the option to send the patient a text. To send the forms, you can use one of the quick options offered toward the bottom right of the panel. Find the option that states Send General / Consent Form.

That will open up a new window where you can select which forms/consents you would like to send. You will see currently pending forms for the patient already listed. To manually select another form or consent you can use the two options at the top. At the bottom are messaging options to choose how it sends, and who to send it to.

If you prefer, you can also access this same feature from any Patient's Information Window. You can search for the patient in the top right of the website, and then select your recipient. With the window open, select the Form tab on the right, and you will see their currently pending forms. Towards the upper left of this panel you will find the same button to Send General / Consent Form.

Custom Tags

For some Consent Forms there can be a Custom Tag in place, which requires more input before allowing you to send the information. These are an optional addition into the consents, that prompts you to fill out the details before sending. For example, if the consent form requires the teeth numbers, you will need to fill that in before sending it to the patient. 

On the form itself, you would see a field like this: send forms 6

Then when sending the form, to actually fill in that field for the patient, you will need to select the outlined icon below.

Once selected fill in the information for each custom tag, then hit send. It will not allow you to continue, or send the form to the patient until this step is completed in the case of a Custom Tag.

Open forms through a tablet

If you have a tablet connected to Practice by Numbers, you can also pull up any forms directly from that device. First, on that device go to app.practicenumbers.com/forms. This will lead to the check in page for patients, but at the bottom you will find a Forms & Consents option. This will pull up the same menu from when you send the forms above. You can select whichever forms you would like for a patient to complete, then hit Send

Before the forms it will ask for some basic patient info so we can link the completed forms to an existing patient, or use the info to create a new patient.
