How to automate the patient forms
Patient forms can be sent out automatically, but consents need to be sent manually as in most states the consent needs to be completed in the presence of the doctor.
To automate the patient forms, start by navigating to the Forms tab at the top of PbN, and then find the Forms option on the left.
From here you will see each of your patient forms, and the setting to determine how often each one is required.
- Manual: Never sends automatically
- Once: Will send automatically for their first appointment
- Every Time: Will send prior to every appointment
- Every ____ Months: Based off the patient's last visit date, and then will repeat after the selected period.
Next is to make sure the message is set to send in the forms settings. You can find the Settings option on the left, then scroll down to find the New & Existing Patient Forms options. To activate them, check the box to the left each and then set the switch to active at the top right of the setting.

Once activated it will automatically send the forms based of the settings you chose for each. The message contains a link, where they can complete each required form together. Once done you can find them under the completed forms.
