Online Forms Kiosk

How to utilize the Patient Forms Kiosk

Grow Package is required for this feature.

Once you have you connected a tablet to PbN, you can use it as a kiosk for the online patient forms. In case you need assistance registering a device, please read the full guide here

With the device connected, all you need to do is open up an internet browser on the device (whichever browser you used to register the device, which is listed under your active tablets) and go to app.practicenumbers.com/forms.

From the landing page, the patient can select Please Check-In, then it will ask for that patients full name to look for an active file to connect to. Once the patient inputs their information, they should see their name listed and can continue. Please note only patients scheduled within the next 2 hours will be able to find their name and continue.

Once they continue, they will see how many forms they need to complete, and will be able to decide where they fill them out. If the opt to fill them out on their phone, it will send a link to their device for them to fill out their information. 

At the bottom of the page it also provides the option to upload a photo/document using the camera on the device which will go to Patient Media

Once completed, you find the submitted forms in PbN under the Completed Forms page, and with the correct settings you will also find them written back into your PMS. 


From the landing page of the kiosk, you will notice the Forms & Consents option available. This allows you to send any specific forms of your choosing to the device, without specifying a patient. Doing so will allow someone to fill out the paperwork even if they are unscheduled or if there is anything additional you want someone to complete. Once done, this will also be found under Completed Forms page, and from that page you might need to manually choose which patient file to sync the information back to.
