Consent Forms Customization

How to create and use consent forms in Practice by Numbers

To view the Consent Forms in Practice by Numbers, start by navigating to Forms at the top, then the Forms option on the left. From there you can select Consents to the right.

Create and Customize

From here you will see a list of each category and consent form currently available to you. You can customize or reorganize the existing forms, or create your own as well. To create a new form, select the +New option.

First, you will want to name and categorize the form.

Then under the Main Consent Text field, you can either type or paste in the verbiage for the consent document. Formatting usually copies over with no issues if you have a Word document with the verbiage. If you are typing directly into the field, we do provide basic formatting options like font, bold, lists, etc. Please note that when the patient receives the consent form, they will only be able to agree and sign, they cannot input any custom information (similar to a read-only PDF file).

Just below that is the Confirmation Text, which is the verbiage the patient will see when checking the box to agree. 

Placeholders / Pre-populated information

Along with the basic formatting options, you will see an option to Insert Placeholders, which allows you to include prepopulated information like name, date of birth, or practice information. When used, you will see the information within the symbols, which indicates pre-populated info. When sent, it will reflect the patient's information. This also allows you to create patient initial fields within the text.

Each of the detected prepopulated fields will also be listed above, so you can know that are working.

Custom Tags

Similar to the Placeholders option we also can detect Custom Tags within the verbiage. This allows you to create fields where you can input specific information prior to sending the consent to a patient. One of the most common examples is the tooth number. To create your own tag, all you need to do is place any verbiage between with an underscore in-between words. For example,

Just like the Placeholders, each detected Custom Tag will be listed above so you can be confident it worked.

When sending a consent form with a custom tag, you will get a prompt like the one below for you to input the information prior to sending.

Connect with Scheduled Treatment

With your consent form created, optionally you can connect it to specific service codes to automatically prompt this form. When you are sending the form, this allows us to automatically select it for you based off a patient's scheduled service code(s). Please note this does NOT automatically send the consent, as in most states the consent needs to be completed in the presence of the doctor.

Staff Signature Settings

By default, consent forms will not require a doctor or witness signature. Please refer to your state's laws regarding patient consents and decide if you want to turn this feature on. By checking one or both of the boxes, we will present the option for the doctor/witness to sign the forms after the patient.

If the patient is using their own device, the consent will be added to the Action Center on the Home page in PbN to be completed later. To sign, navigate to Forms and then Completed Forms on the left. From here you will see recently submitted forms, and the option to sign for the consent.

Sending the Consent Form

This article goes over how to send forms to your patients, either to their personal device or to a tablet at the practice. As a reminder, the consent forms can not be sent to the patient automatically as in most states the consent needs to be completed in the presence of the doctor.
