This video will guide you through creating a custom form, explaining the components/widgets that should be used and when. Below, you will also find a list of the components available and how they can best be used in your form. 

 

Forms Blocks

  1. Header: A block that displays a title or heading at the beginning of a section or the entire form. It typically provides a brief description or summary of the content that follows.
  2. Selection Header: Similar to a header, but specifically used to introduce a section where the user is expected to make selections or choices from a list of options.
  3. Paragraph: A block for displaying text content, typically used for longer descriptions, instructions, or comments within the form.
  4. Image: This block allows users to insert an image into the form. It's often used for visual elements such as logos, illustrations, or diagrams.
  5. Text Box: A block for inputting short text or alphanumeric characters. It's commonly used to capture names or other concise pieces of information.
  6. Number Box: This is similar to a text box but specifically designed for entering numerical values. It includes validation to ensure that only numeric input is accepted.
  7. Text Area: This block provides a larger area for entering longer text content compared to a text box. It's suitable for capturing comments, notes, or other more extensive information.
  8. Select: Also known as a dropdown menu or list block, this block allows users to choose from a predefined list of options. This can include an 'other' option where text can be input and can also be set as a multi-select option.
  9. Checkbox: A block that presents users with a binary choice, typically for consenting/agreeing to within a form.  
  10. Radio Buttons: A block for selecting one option from a list of mutually exclusive choices.
  11. Date Picker: A block that provides a calendar interface for selecting dates. It ensures standardized date input and helps prevent formatting errors.
  12. Signature: This block allows users to provide a digital signature.
  13. Yes/No: A block presenting users with a binary choice between "Yes" and "No," commonly used for simple confirmations or decisions.
  14. Terms: This block displays terms and conditions or legal agreements that users may need to accept before proceeding with the form.
  15. Image Upload: This block allows users to upload images from their devices. This is typically used to collect images of an insurance card.
  16. Medications, Allergies, Diseases, Referral: These blocks capture specific medical information related to a patient's medications, allergies, diseases, or referrals to other healthcare providers. The items displayed in these blocks are linked to the active items in the Forms→List page.
  17. Address: A block designed for entering a physical address, including separate fields for street, city, state, and postal code.
  18. Payment Method Block: This block captures and saves information related to the patient's preferred credit card details. The Payment feature must be turned on for this block to operate correctly. 
  19. Patient Name Block, Gender Block, Date Of Birth Block, Patient Profile Block: These blocks capture essential patient information such as name, gender, date of birth, and other details relevant to their profile or medical history.
  20. Separator: This block visually divides sections or groups of fields within the form, helping to organize and structure the content more effectively.
