Eaglesoft Medical History Form

How to use the Eaglesoft Medical History Form within PbN.

Eaglesoft practices will now see a new Medical History Form under Forms built to mirror the Eaglesoft Medical History Form and make it easier for information to be written back into the Patient's chart. 

Here is the information found on the form.

The items from the Eaglesoft list blocks will create alerts in the Patient's chart. Other fields that are filled out will be present in the form only.

Here is an example form that has been filled out. The items marked in red will create an alert. (Right-click and open in a new window to enlarge)

Here in the Patient's chart are the alerts for the items chosen on the Medical History form above.
